Chiropractic Paperwork Process:

At Locklin Chiropractic we give our patients the quick and easy option of filling paperwork out online. We are able to send the link straight to our patients phone numbers or emails.
​
If for any reason you are unable to fill out the paperwork online, we do offer patients the option to fill it out in the office before their appointment. We just ask that you allot 15-20 minutes in order to fill out the paperwork before the time of your appointment.
It is our policy to have returning patients update their paperwork. If it has been more than 2-3 years since your last visit you will be sent our online patient paperwork to fill out unless you request to fill it out prior to your appointment in the office.
