At Locklin Chiropractic we take patient privacy seriously; We understand the importance of protecting and safeguarding all personal information.

This Notice of Privacy Practices (NOPP) explains the array of personal information we collect, why we collect it, and the steps we actively take to protect it here at Locklin Chiropractic.

What information do we collect?​

All staff and providers are subject to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). If you are currently a patient (or looking to become a patient) then all of your protected health information (PHI) we collect during and after the course of your treatment is covered under this Act.

  The information we collect includes:

  • Administrative, Demographic, and Insurance. 

- Patient identifiers such as names, addresses, and contact information.

- Demographic data such as age, gender, ethnicity, etc. 

- Insurance information such as insurance card details, primary card holder data, details about insurance coverage, and payment sources.

  • Health Information

- Information about past and present illnesses, conditions, and/or any treatments. 

- Medications ( prescribed and over the counter)

- Information on any surgeries, and tests such as X-Rays, MRI's, Blood work, and Scans. 

-Vital signs such as blood pressure and heart rate. 

- Treatment records or any other documentation of care provided by other healthcare professionals.

  • Service Utilization and Cost

- Information on the types of healthcare services accessed. 

- Information on the cost of services provided, including billing and reimbursement details.

Why do we collect this information?

Personal information is crucial in healthcare to effectively diagnose, treat, and continue ongoing care. It allows Doctor Locklin to provide a tailored treatment plan to a patients individual needs, monitor progress, and ensure patient safety.

How do we protect this information?

Here at Locklin Chiropractic there are many practices and procedures the in office ( and out of office) staff perform while handling any and all patient information. 

  Practices and Procedures:

- All staff are required to sign a HIPAA agreement prior to the start of their position.

- All office staff have received their certification in HIPAA training and are subject to these trainings annually. 

- All paper copy files and patient information are stored securely within the office. 

- All inactive files are dated and set to be shredded after six years based off the patients last Date Of Service. 

- Redaction methods are used within the office. 

- A shredder is utilized daily for any unnecessary paperwork or paper copies. 

- Access controls such as passwords, encrypted software protection, automated backups, and unique user ID's in regards to electronic data and information. 

- All sensitive files or information required at the desk is not left unattended and is always placed upside down. Therefore no patient information is visible to public view. 

- Private rooms are utilized for patient privacy during appointments.
