Service Description

We do require all new patients to fill out the necessary paperwork prior to their appointment with us. For this service we only offer a paper form which may require us to ask for your home address in order to send it to you. However if we do not have enough time to send the paperwork in the mail we just ask that you arrive 25-30 minutes early in order to fill the out the paperwork.
