Welcome to the Locklin Chiropractic website. By accessing the Locklin Chiropractic PC website you accept and agree to be bound by these terms and conditions of this agreement. If you do not agree to abide by the above, please do not use our website.

Use of this website does not establish a doctor-patient relationship between you and Locklin Chiropractic or Dr. Locklin. A doctor patient relationship is only established through an in person examination and/ or consultation.

Locklin Chiropractic takes protecting patient health information very seriously. We are committed to adhering to the Health Insurance Portability and Accountability Act. All protected health information (PHI) submitted through this website or in the office will be handled in accordance with our Privacy Policy ( Read More Here) and HIPAA regulations.

Patient Responsibilities:
General Health: It is the patients responsibility to provide us with accurate history and to update us with any physical or medical changes.
Billing/ Insurance: It is the patients responsibility to provide us with up to date insurance information. IF any changes are made to a patients insurance, or address, names, phone numbers, etc. It is the patients responsibility to provide us with that updated information. We will submit claims to your insurance company ( if applicable), but you are responsible for all charges regardless of insurance coverage.
Appointments/Cancellations: It is the patients responsibility to arrive to their scheduled appointments on time. IF it is necessary for you to cancel your appointment then it is our policy that you provide us with at least a 24 hours notice. IF you fail to contact us 24 hours prior to your appointment then you may be subject to a $45 No Show Fee.
Online Appointments:
We offer the ability to schedule appointments online through our website. You are not scheduled for an appointment with us until you receive a confirmation message from us. We work to approve all appointments made online as soon as we are able to.
Payment: All payments are due at the time of your service unless other arrangements have been made. At Locklin Chiropractic we accept the following insurances: Anthem, Excellus Blue Cross Blue Shield, Federal Blue Cross Blue Shield, Standard Blue Cross Blue Shield, United Healthcare (Empire Plan Only). We will submit claims to your insurance company as a courtesy, but you are responsible for all charges regardless of insurance coverage.
We are currently in the process of phasing out insurances and are no longer accepting new patients with the following insurances: Aetna, Humana, Medicare, Medicaid, Fidelis, Wellcare, Today's Options, Cigna, United Healthcare (with the exception of the Empire Plan), Personal Injury, and Workers Compensation.
We offer an Out-Of-Pocket Rate for those who either do not have insurance, or have an insurance we are non-participating with/ no longer accept.
We accept these forms of payments: Cash, Credit Card, Debit Card, HRA/HSA Cards, and Checks.
No Show/ Late Cancellation Policy:

Effective as of February 2022 any established patient who fails to show or contact our office with at least a 24 hour notice to cancel/reschedule will be considered a NO SHOW and charged the full standard rate of $45.

Any new patient who fails to show for their initial visit will NOT be rescheduled.

The fee is charged directly to the patient; NOT the insurance company and will be due by the time of the patients next visit.

Repeated no shows may result in DISMISSAL from the practice.

As a courtesy, when the time allows we make reminder phone calls for our longer appointments. If you do not receive a call the policy still remains in effect.

We ask that all patients confirm their appointments with us when we make the reminder phone calls. If we do not reach you we will leave a short message stating your appointment date, time, and ask that you return the phone call to confirm your appointment. If the appointment is not confirmed then we will begin scheduling other patients in place of your appointment.
We understand there may be times when an emergency occurs and you may not be able to keep your scheduled appointment. If you experience extenuating circumstances please contact the office, we may be able to waive the no show fee.

All staff and providers are subject to the Health Insurance Portability and Accountability Act of 1996 (HIPAA). If you are currently a patient (or looking to become a patient) then all of your protected health information (PHI) we collect during and after the course of your treatment is covered under this Act.

How do we protect this information?
Here at Locklin Chiropractic there are many practices and procedures the in office ( and out of office) staff perform while handling any and all patient information.
Practices and Procedures:
- All staff are required to sign a HIPAA agreement prior to the start of their position.
- All office staff have received their certification in HIPAA training and are subject to these trainings annually.
- All paper copy files and patient information are stored securely within the office.
- All inactive files are dated and set to be shredded after six years based off the patients last Date Of Service.
- Redaction methods are used within the office.
- A shredder is utilized daily for any unnecessary paperwork or paper copies.
- Access controls such as passwords, encrypted software protection, automated backups, and unique user ID's in regards to electronic data and information.
- All sensitive files or information required at the desk is not left unattended and is always placed upside down. Therefore no patient information is visible to public view.
- Private rooms are utilized for patient privacy during appointments.
This Notice (NOPP) describes the Privacy Policies of Locklin Chiropractic, PC that was updated on August 11th, 2025.
Limitation Of Liability:
The patient acknowledges and understands that chiropractic adjustments, physical therapy, and other therapeutic procedures involve risks inherent in manual manipulation. These risks include, but are not limited to, muscle soreness, temporary exacerbation of symptoms, sprains, and in extremely rare cases, bone fractures or vertebral artery dissection. In the absence of gross negligence or willful misconduct, the Clinic, its owners, and its licensed practitioners shall not be liable to the Patient for any indirect, incidental, special, or consequential damages. To the fullest extent permitted by applicable law, the total aggregate liability of the Clinic and its agents for any claims, losses, costs, or damages arising out of or related to the services provided shall be limited to the total fees paid by the Patient for said services in the preceding twelve (12) months. The Patient agrees that they are ultimately responsible for the safe and continuous operation of their personal health journey outside of clinic visits. The Clinic makes no warranties or guarantees regarding the absolute outcome of any chiropractic treatment plan. If any provision of this limitation of liability is deemed invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.
We are not responsible for any health consequences resulting from your use of information on this website or delay in seeking professional medical advice.
